Church Health Services, Inc. Mission:

Church Health Services, Inc. is a faith-based healthcare organization that conducts primary, dental and mental health care clinics for the poor, uninsured, and underinsured, and partners with local congregations to help them become centers for health and healing.

We have partnerships with the Beaver Dam, Waupun and Horicon School Districts to provide mental health counseling to children in need with appointments taking place in the child’s school building or at our clinic.

Our dental and mental health care services are for both children and adults.
