Founded by volunteers in 1990, Rainbow Community Care provides services that support patients and their families throughout the entire journey of aging and serious illness. The organization is highly regarded for its excellence in hospice care, palliative care, advance care planning, grief support, parish nursing, and volunteer programs. They care for individuals in their homes, assisted living facilities, nursing homes, and at the Rainbow Hospice Care Inpatient Center in Johnson Creek.

Rainbow Community Care is dedicated to improving the overall health of the communities it serves in rural southern Wisconsin. Its daily work is guided by a set of values focused on caring for the most vulnerable members of the community. The organization is committed to supporting individuals with the core belief that this is their life and their journey.
